Community Health Partnership Seed Grants Available!

NBCH is pleased to announce that applications are now being accepted for the NEW Community Health Partnership (CHP) Population Health Seed Grant Program.

Presentation: Expectations for Proposals 02/26/10

Goal: The CHP Seed Grants will increase the number of business led coalitions working with public health and other stakeholders to improve the health of the population in their communities. The seed grant program will provide support to such collaborative efforts. The grants are intended to promote implementation of promising and evidence based initiatives in the field with a diverse range of approaches and applicants.

Available Funding: NBCH, through its nonprofit affiliate organization the Community Coalitions Health Institute (CCHI) will be awarding at least $200,000 to 5-7 coalitions. Grant amounts will vary depending on the coalition’s proposed budget and needs. The maximum grant is $40,000, but smaller proposals have a higher probability of being funded. NBCH expects to fund a varied group of member coalitions reflecting diversity in geography and coalition size. Additional funding may become available in the future. Applications not funded in this round may be funded in the future if additional dollars are made available.

Who: Eligible applicants must be NBCH member coalitions in partnership with their employer members and local and/or state governmental public health officials (at a minimum). Preferably coalition applicants will have engaged other community stakeholders as well. 

What: This grant program will support collaborations and partnerships of people and groups taking actions to improve health status in a defined population. Projects may address development of a program (such as a community health promotion initiative) or a policy (such as a healthy vending machine or healthier menu initiative). Grant applications must address health promotion or prevention relevant to diabetes, respiratory conditions, or any public health indicator used in the America’s Health Rankings^® tool. Where: NBCH, through its nonprofit affiliate CCHI, will award grants in diverse communities and to coalitions of all sizes and experience levels. NBCH/CCHI will award grants for CHP initiatives to improve population health in no less than 5 communities initially, and may add additional communities on a rolling basis.

When: Applications are due March 30, 2010. Funding notifications will be made by April 30, 2010, and projects should be started by June 1, 2010. The project period will be June 1, 2010 – June 1, 2011. Final reports will be due by July 30, 2011.

More details: Regardless of size and experience, the coalition must demonstrate in the grant application that it has the capability to carry out the project as proposed. Funds will be released based on achievement of milestones identified in the proposal or implementation work plan. Some technical assistance will be available from NBCH and its CHP partners. Applicants submitting proposals lacking a specific project plan will be offered technical assistance and invited to resubmit for future grant offerings.
